Declaration for the Uniform Consumer Credit Code and Disclosure Information and Privacy Act Acknowledgements

I/we declare that the credit to be provided to me/us by Unisus Group Pty Ltd (including any of its related bodies corporate or associated businesses) (herein referred to as “Unisus”) is to be applied wholly or predominantly for business and/or investment purposes.

Important: You should not submit this application unless this credit/loan is wholly or predominantly for business and/or investment purposes. In submitting this declaration you may lose your protection under the consumer credit code.

I/we consent, authorise and understand that Unisus (including any of its related bodies corporate or associated businesses):-

May give to and seek any credit providers that may be named in this credit application and any credit providers that may be named in a credit reporting agency information about my/our credit arrangements; and

That this information can include any information about my/our credit worthiness, credit standing, credit history or credit capacity that credit providers are permitted to give or receive from each other under the Privacy Act 1988.

I/we understand the information may be used for the following purposes:

• To assess an application by me/us for credit

• To notify other credit providers of a default by me/us

• To exchange information with other credit providers as to the status of this credit application/loan where I/we are in default with other credit providers

• To assess my/our credit worthiness

Recovery of Legal Costs and Disbursements and Collection Expenses in Case of Default

I/we agree to be bound by the terms and conditions of sale as referred to overleaf, disclosure of information under the Privacy Act 1988 (cth), declaration under the uniform credit code as set out in this application, and warrant that the information given by me/us herein is true and accurate.

I/we agree that interest, at ruling bank overdraft rates and collection charges may be charged on overdue accounts.

I/we agree that payments shall be made within 7 days from the date of invoice and, if at any time, monies are overdue, the whole account then becomes overdue and your company is justified in proceeding legally to recover the whole debt including any and all legal costs and disbursements incurred by Unisus on a solicitor/client basis, and any collection expenses incurred. We agree and acknowledge that such collection expenses do not constitute a penalty but Unisus’s genuine and anticipated expenses incurred in the event that legal and/or collection action is commenced.
